Update: Since Dr. Chuong explained the thought process behind the things I found odd, I raised my rating. Most of the doctors I see request my license and insurance information at the time of the actual appointment. I usually provide the basic details over the phone so they can verify my insurance before the appointment. The only time I have ever had to text my insurance card was to a dental office and that was after I made the appointment.
