You're get Nurse Practitioner at these Conviva offices instead of an actual doctor; not taking away from NP's but your insurance and out-ot-pocket costs, if applicable are billed as if you are being attended to by an actual physician. Personally, I left Conviva because of this and the fact that at last visit, the NP told me to call whenever I needed refills on my prescriptions and when I called it was very difficult to get refills.
