Agent failed to procure homeowners insurance, failed to use due diligence in locating a policy, failed to bind coverage on new home purchase, failed to change address on existing vehicle/life policies, failed to submit a change of agent form to vehicle/life insurer after a year and a half causing the policy to be cancelled.
