Don't expect to get in on time, or any customer service I have been to this place twice now, only because I bought a groupon. Both times, my appt started 15 minutes late (although I arrive on time). There has never been any apology or acknowledgment of their tardiness. Their form asks if you are allergic to sulfa, yet nobody could provide any further information. One snafu: Their information form has a mistake, as one question asks for "you're" name.
