I paid Dawn Martin of Chatfield Legal Document Service in April 2018 to form an LLC for me and to provide me with the document I need to place my business into the LLC. The LLC was approved in September 2018. As of February 2019 she has not provided me with the last document I need. I have contacted her numerous times asking for this document.
