First off I'd like to say Dr. Chase and his staff were very friendly. Unfortunately, they no longer have a provider under Oxford UnitedHealthCare to make appliances even though their office accepts this coverage. I found this out AFTER a day of of commuting into Manhattan and spending over an hour in their office for a consultation and mouth impressions.
