Plain arrogant doctor and doctors upper management of the clinic, would not accept FMCSA DOT vision evaluation form, which is a new requirement in place of the old vision waiver form from DOT, This new rule is in affect since March 22, 2022 and yet they fail to acknowledge with proof of email from DOT and their website, still telling me wait we need to investigate, I even called and verified, because of their arrogance people have to lose time would not recommend, now I have to go to another office because of their arrogance... read more
