I can’t say enough great things about Care Resources and Melissa Kuttas! I have been a client for close to one and a half years. This company is wonderful to work with. They are very resourceful and great advocates for our patients. Heidi Elpers has been great on continuity of care for patients transitioning from different levels of care. Recently this year she helped a patient transition from a home setting to an assisted living facility. She coordinated getting items removed from the patient's home, moving items to the assisted living, and coordinated medical care to continue at the assisted living facility.
