Woodbine Nursing Home in Alexandria, VA on King street has many reported violation by the Richmond Commissioners Inspection office for breaching accreditation standards. My Great Aunt died there in 2011. My mother (age 80) has been there 4 years and she is now in the Emergency Room at Alexandria Hospital.
