My daughter needed to have a medical procedure done and went through all the correct channels with the insurance company to have the surgery, and was told twice from the insurance company that her procedure would be covered. All the pre op tests were paid for by the insurance company prior to her surgery. Once my daughter had the surgery her explanation of benefits started arriving from the insurance company reversing all claims.
