I've been trying for nearly a half hour to call this facility to learn if my 94 yr. old mother has been transferred here from Lake West Hospital in Willoughby, OH. Each time I call, it rings 10 times, then a recording tells me that no one is answering to try back later. The hospital told me she's no longer a patient there. She was scheduled to go to this facility, but before using the gas for a 22 mile round trip, I wanted to verify she was actually there. This is a great place. I have had 3 different loved ones in this place for therapy. The care they gave was good. I was always kept informed of new issues (medication changes, changes in health care concerns). The Therapy department did a wonderful job working with my family members. The nursing staff was great! The DNS is amazing; she makes herself available to everyone at all times (I don't think she ever takes a day off, LOL). The food is not as good as "home cooking", but it is satisfying and well-balanced.

The staff is very friendly and helpful. Always trying to go out of their way to take care of the patients. Mrs. Baker is the best!!

My mother has been in this facility for about seven weeks. No other facility was able to take her, but Annandale Healthcare had room for her. My mother is dying and also has shown signs of dementia which means she has good days and bad days. The staff has been amazing with her even when she has been extremely combative with them. The level of care she is receiving is wonderful and it has been nothing but a blessing to our family to know that she is receiving the best care possible. Do not take your loved ones here! Nurses and Aides physically, verbally, and mentally abused my father in a couple of months time. He is a VETERAN and has aggressive tremors in his arms/hands. He cannot care for himself at all, nor can he protect himself from abusers. There was an investigation done for his abuse and the aide was not allowed back into his room. However, she is still there to harm other patients! After a complaint was filed with ombudsman, my father was threatened that his food would be ground up and tube fed... read more

Do not let loved ones go here. Three mos I have gone w/o my sleep med or anxiety meds. Was told they, as a result of defacto practice do not prescribe Ambien, no real reason given, nor was I offered alternative. A policy they go by without ever having a problem receiving it in hospital, or at home. Cold turkey, discontinued anxiety medication, told I wasn't taking it at hospital, nor at home. 3 months later, I was given a dose 1/4 the dose I've been on for 2 yrs. I was recently released from Advanced Specialty Hospitals of Toledo Ohio after spending 25 days in their care. I have been in a number of nursing homes and hospitals over my 87 years but none of them came close to giving me the excellent care to the point of making me feel like I was a part of their family .
