Simply need a PCP and IPA number BlueCross Blue Shield's form requires. Called there 1-800 six times only to get empty platitudes, insipid identification verification questions, endless transferring from one dept. to another resulting in being hung-up on and then asking for a survey... More fun than going to Disney!!!
