Beware!!! I went in for a well person visit (which is free under my insurance). They took an extensive history, then billed me almost $200 because they refused to code it as a well person visit. when I requested my records so I could go to a different provider, the records were full of errors (diagnoses I don't have; allergies incorrect; & a wrongful accusation of family physical abuse). Insurance company has repeatedly asked them to change to code and they refuse.
Chase took care of my mom at home...he was very reliable and great with my mom.
Please be advised that a nurse practitioner there misdiagnosed my husband. He was told it was ....and it was actually cancer. He passed away in January 2025. Had he known and been treated properly, He might have had a chance. So be careful. If you question her decision, please get a second opinion. So disheartened with this so-called medical professional. Scary!!
