Went in for a procedure where my doctor used their services. Of course they did their job for the doctor, but when it came time for the billing never received a bill. Then two months later received a phone call from them asking for my apartment number, because they never got that info. Was told I would receive a bill in the mail shortly. I waited and forgot.
