I would like to provide feedback regarding the current structure and communication practices within the Transitional Care Unit. While the clinical care provided appears appropriate, there is a noticeable lack of inclusion of family members in patient scheduling, care planning and progress updates. Family involvement is a critical component of successful transitional care, particularly for patients who require structured support, advocacy or assistance with decision-making.
