I needed a form filled out and faxed to my insurance. I was told that the Doc would make me make an appointment (of course MONEY) I did I had to see this Doctor Clark He told me he was going to fill out the paper and do everything he could to help me at the end of the visit he also said let me check you breathing so I can say I did something. It took me almost a week after numerous calls from me and my insurance provider to get it faxed. I then needed Clark to call on my behalf to give them more info and he refused
