Got a bill while in a medical facility where it shows Dr. C evaluated me then visited 7 separate times on different days and billed my insurance for it even though I only met her once. Insurance has called multiple times and physically mailed letters requesting more information and never got a return phone call or message. The location now shows to be permanently closed but they keep sending me bills somehow. Insurance is refusing to pay because they keep requesting information and nobody responds.
