I went through my medical records in the BayCare portal today, again, and found that my "History and Physicals" report from October is now replaced with an Addendum to a surgical report for an older woman. The location reads as "BMG SUR CBCC T", which I imagine is this place. It has the patient's name, but no birthday. MY name & birthday are on every page, but again, it is not my actual medical info.
