Highly unusual financial business practices. Our daughter was overcharged her copay 15 times over six months without the company noting it or stopping the overcharging or initiating any refund. When we found the error, and called their attention to it, we were told "You were on our outstanding refund list and I'm sorry it just hadn't come to my attention", which translates to we might have given you back your money had you noticed it but otherwise were going to continue using it indefinitely.
