Unfriendly staff. Ordered a large milkshake and it tastes watered down with bits of ice in it. Is ice cream shake that's worth $12 really supoose to have crushed ice in it .?

I frequent this DD/BR and half of the time the drinks are decent. The staff is a little slow so I would expect a wait. When ordering ahead, make sure to check your order, they never know to pack the donuts until you ask. We went for the BR 5/31 deal which was 31% off your purchase. Never in my life of ordering ice cream have I been charged for the cup. There was nowhere on the menu where the cup was said to be an extra charge. What am I supposed to do?
