Edit to address response from practice: this was a typed action plan with signature on the initial form from your doctor in conjunction with prescriptions they gave. Patience required is understandable, doctors not having any record of medical advice they gave patients is not, and it’s a liability-not to mention gaslighting saying I was wrong and it didn’t exist. A simple phone call saying I’m sorry we can’t find record of it can we talk through what this is, would have solved everything and been completely understandable. I... read more
