The management here is absolutely horrible. The admission director Donna is the worst. She is dishonest and never return calls. I'm sure I'm not the only one that has experienced this lack of communication and care while trying to get information for a loved one. I wish I could leave zero stars!

My mother was at this facility for 6 months for rehab following a femur fracture. As a medical professional, I never fully appreciated the importance and dedication of rehab services. The PT and OT staff worked diligently with her to get her mobility better than it was prior to the fracture. They never gave up on her and supported her every step of the way!! Firstly, I am not one who posts a lot of reviews. That said, my loved one spent nearly three weeks in this facility and for the most part it was a good experience. Like anything or anyplace that is taking care of your loved one, you need to be their advocate and be there to assist in their care if you can. The people for the most part have everyone's best interest in mind and try to help. Sadly, there are always a couple of exceptions. My brother was admitted there yesterday and he fell down from the bed twice in less than 24 hours. He died there in less than a day! They don’t provide 24/7 care at all!! Please don’t send your loved one there… unless you want him/her to die sooner….😓😓🥲🥲

Autumn Care great facility and great staff would highly recommend family and friends. Staff was very courteous and professional
