After I spent over an hour to fill in all information on their web site. Got a call from them accused me not filling out the medication and surgery information. I tried to explained that I already filled it int, the girl refused to hear me and kept saying that I did not fill it in. She said she did a merge of my information that could cause loosing my information. Instead of asking me on the phone for more details she said she could not book appointment for me.
