I always take my son for his annual well child check and do his sports physical at the same time. I have 4 kids so the fewer appointments the better... until this time... they charged me $30 to fill out the physical paper that I brought. I was super bummed that they started charging. I understand if I was making an appointment for a physical only but it was with his well child check. Honestly, I'll probably skip his next WCC and get his physical done somewhere cheaper next time.
