Home care was to include Home Health Aide, PT, , and OT along with RN visits. RN did visit; but, the other services never started. 12 days after discharge no phone calls from PT or OT. Home Health Aide had a date to start but no phone call or visit.
My Father received excellent and compassionate care. I was completely pleased and wound use Ascension at home for all of my family.
