I went to get Botox for the first time at Arndt Aesthetics in March. As a new client, I thought they would ask for medical information but all they had me sign was a waiver. The Botox wore off within 1 month and I felt uncomfortable going back so I went to another provider. The new provider asked for a complete medical history regarding medications or medical conditions.
