I was discharged from rehab on Friday July 17th. I was told that Aquinas home health would be contacting me. If I had not heard anything by the following Monday, I should contact them. I did and was told I would be evaluated. On Saturday the 25th of July a nurse came out to evaluate me. I heard nothing else until Wed. the 29th when a man came out and did an additional eval.
