The absolute worst and must disorganized dr ever. We have been going back and forth about a physical form for two weeks that has not been signed. Multiple new faxes have been sent to dr to restart the process. Finally got a call to pick up today and drove 20 minutes there to find out it still is not signed and she won’t be in until Wed. Other friends have had same issue and have left for other drs. Save yourself the time and go somewhere else.
