I made an appointment specifically for a physical and gave the office my insurance info. When I got there, they accepted my copay. Dr. Anderson didn't perform a physical, merely did a "meet and greet," which I'm pretty sure could be done at the same time, but whatever. So basically I received no service. THEN the office decided that no, they don't take my insurance after all and billed me $100. So basically it cost $100 because they made a mistake, and I got no service out of it.
