This is a yes or no question, you don't need anything but the name of the plan, and I verified it with the insurance company. YOU DO NOT NEED PERSONAL INFORMATION TO ANSWER THE QUESTION. This is per the agent I spoke with at United health care. I'm not trusting strangers with all my personal medical information. Do the world a favor and go out of business.
