The form asked what the injury was, how long he would he off, etc. The provider refused to sign it. But kept the form and wrote exactly what the form asked on a tiny piece of paper…. Very busy and extremely bad staff in the front. They put you on hold for hours and hours and looks like they are running the show and doing things behind the Dr back. Try getting hold of the Dr to tell him anything....good luck!! If you are worried about ur health I don’t recommend this place.
