This is the worker's compensation carrier for my employer. Though my employer's corporate HR representative called to inform me I had a valid case, this company was less than professional in their handling of same. Definitely have an attorney handle your claim, as that was the only recourse in my opinion.
First this location is not an insurance agency, it is the regional office of a large international insurance company. This just goes to show how utterly useless this information can be.
