Truly grateful for the compassionate and knowledgeable care my grandmother received in the last few months of her life!! The team at Activa are incredible, starting with the strong leadership of Kirstin and Kelly to the scheduling coordinator Dawn, and case manager Hazel. All the clinicians that were in my grandmother's home provided exceptional care. Our family is truly grateful for the team!!

I can’t say enough good things about Activa Home Health and the amazing team behind the scenes. Every time I called with a question or concern needing to be addressed the staff- Kelly, Hazel, and Dawn went above and beyond to make sure I am supported, informed, and reassured. I have a fractured right kneecap and 2 fractures in my left wrist. Chris and Alex were my physical therapists and Nicole was my occupational therapist. This team of therapists is exceptional! They communicated with me in a timely manner. They were very encouraging and showed me how to improve with each visit. They are very professional and personable at the same time. I would give them 10 stars if I could. I recommend them highly. I am extremely pleased with my progress. 🤗
