Upon being discharged from Riverside Hospital after foot surgery, a COPC transitional care nurse put me in touch with A Place At Home which turned out to be the luckiest day of my life. After spending a week in a hospital bed the last thing my husband wanted was to put me in another institutional or rehab type place.
