I am in rural San Martin CA. 21st Century Home Health provided me with Home Healthcare services including nursing, physical and occupational services. Their staff is great. I was served by Dong the case manager for PT & OT. Tuyet the PT, Jillian the OT. Kristina supervising RN, Steven the LVN and Rachel the Home Health Aid. They are all compassionate and very helpful. Nurse Nancy has done a fantastic job in treating and bringing to a point of healing a very serious back wound experienced by my nearly 98 year old mother. The process took several months and Nurse Nancy was there every step of the way with her expertise. Patient Experience Coordinator, Shawnt, has overseen the coordination of Mom's care with both her nurse and PT and is appreciated for assisted in keeping care running smoothly and being available.
Krizza and Ana were our nursing team and they are fantastic. Professional authentic caring and attentive. Could not have asked for better care. Post op patient with one post op complication. All the best to these fine young women who are excellent nurses and caregivers. Cares for us like family. Thanks ladies. You are the best😁
