I have never in my entire life been to a Dr office that reschedule your appointment if you don't have your new patient paper work filled out, this was not told to me before i went to my first visit but to take off from my job and spend $10 on printing only to get there and be told can not be seen due to not having paper work finished . The worst experience ever after waiting 2 months to see him.
