Walmart Vision Center informed me on my initial visit that my insurance does not cover eye exams, lenses, frames... My insurance provider was shocked as I do have coverage and sent me a copy of my policy showing eye glass coverage. My insurance company requested me to get a copy of the bill including tax id of both Walmart Vision Center and the Dr that performed the eye exam to attach to an Out of Pocket Reimbursement Form since I had paid and Walmart did not bill.
