I pay $2,300 a year for this coverage, my husband tore his bicep and rotator cuff, had surgery and therapy returned to work 6 months later. I filed my claim with Loomis Company for coverage of accident and disability. I sent a breakdown cover sheet with dates, names and cost with copies of every visit and procedure.
