Lack of communication and interest.
We use The Hultquist Firm as our accountants for our three businesses (J. Jones Photography, City Scape Winery, and my wife's real estate business). Mat and his entire team are very down to earth, and put things in easy to understand language - which is saying a lot in the accounting world. Even though we have a more challenging tax situation than most he is very easy to talk to and takes time to explain things when we have questions.
