I was told that SRDA provides transportation to medical appointments and shopping and other services. However the receptionist informed me that they don’t come where I live bc she doesn’t know where we’re located. We don’t drive. The receptionist then tells me that I need to come in and fill out an application. I asked her how I was going to get there. My bathroom floor was still dirty. I text my provider and asked if she mopped. She said she did. I got a wipe and did just a small area and it was really dirty. I sent her the video. I get a call from the office saying they didn't provide the services I wanted which was for her to mop on her hands and knees. I never asked her to do that and never would.
