Recently I moved my mother out of this facility. Specific service plan protocol that the director and staff nurse agreed were important were not implemented. No follow through with staff. The individual care givers were very kind and I believe doing the best they could with poor leadership. My mother made several trips to the hospital in less 2 weeks.
