I recently needed transitional care for a loved one being discharged from the hospital. The hospital gave me a list of facilities to visit prior to his discharge. I visited a few and then met Dania at Providence St. Elizabeth Care Center which is now Toluca Lake Transitional Care. We toured the center, and I was quite impressed with the facility. Before my release from hospital my doctor recommended this place. He said it was under new ownership and thought I would be happy here. He was right. All the staff was been great. Very caring and friendly. You can tell they enjoy their jobs. They have lots of activities available if you want to join or not. I especially like the PT and OT. They really work you out and concentrate on your goals when was it 10

It’s amazing that in under one week I went from an extended hospital stay to home health care, but it’s true! I credit this quick transition to the team at Toluca Lake Transitional Care (formerly Providence St. Elizabeth Care Center) in North Hollywood. Sincere gratitude for all the staff at Saint Elizabeth’s, former Toluca Lake. This is a facility where you can feel confident that your loved one will be cared for. From all the facilities my mother has had to be placed in, none compare. Thank you for your professionalism and compassionate care.
