I visited Dr. Pauline N Ignacio at Physical Medicine and Rehabilitation Electrodiagnostic Medicine. Dr. Ignacio was very patience and caring. I have been having pain for quite a while and know one seem to be able to figure out why, but Dr. Ignacio help solve the mystery. She is thorough and know how to calm patients like me.

I FINALLY FOUND A DOCTOR IGNACIO WHICH CARES ABOUT MY NERVE DISEASE? AND IT'S VERY AWARE OF IT.SHE IS PROBABLY THE BEST DOCTOR I HAD OUT SIX, DESPERATE PAIN CENTERS IN THIRTY YEARS

Made appointment with Dr. Yunis to have a VA form 21-0960M-14. Was told that the form would cost $200.00 to have this form filled out which normally costs $600.00 to insurance companies and businesses but Dr. Yunis said he would discount it to $200. I agreed to that cost and had made several visits for various tests and examinations. I was told that my form would be done today (06/15/18 so went to office to pick it up.
