Made appointment with Dr. Yunis to have a VA form 21-0960M-14. Was told that the form would cost $200.00 to have this form filled out which normally costs $600.00 to insurance companies and businesses but Dr. Yunis said he would discount it to $200. I agreed to that cost and had made several visits for various tests and examinations. I was told that my form would be done today (06/15/18 so went to office to pick it up. Dr Yunis and his Staff work hard to take care of patients issues. I have only had good experiences with them all. Visits with short wait times and a knowledgeable doctor.
