Absolutely horrific service. I follow the forms, and am confident I booked a ride correctly, only to find out days later they did not ensure we had booked a wheelchair ride as requested. Changing it is at least a 20 minute conversation with a bot, until you can actually get to a representative, and then it's another 15 minutes to get it right. If I could I would put ZERO STARS. Modivcare has the worst service I ever had with any transportation. I use them two time the first time I was pick up to my appointment ok then pick up to be taken home OMG they had me drop off at my PO box not near my pick up address and the driver said that's the address he was given now on May 2nd 2025. I had to go have a biopsy very nice driver and I wish Modivcare would use them but I just verify and they don't now when I was suppose to be pick up and requested a transportation to be ab... read more

ZERO STARS. You fill out the information on the Modivcare website, go through hell explaining in detail why you need a ride and then when you hit Submit it tells you the site is down and try again. Happened four times. And forget phone service. Geesh. Actually better to spend the cash and call Lyft yourself. It seems these drivers are not associated with the company so you take your chances for a free ride that you are entitled to but you can't get and you will be late for your appointment. DO NOT USE. Undependable company. I have utilized this company every 2 weeks for transportation to the same wound care doctor. Two weeks ago I booked another ride for today Dec. 19th. Yesterday the day before my appointment I get a text from them saying my ride was canceled due to the vehicle not having a mobility ramp. After being on the phone with them 45 minutes yesterday they finally scheduled another ride. Today of my appointment guess what, they sent a vehicle without a ramp. DO NOT USE. Undependable company. Had doctors appointment end early, so I called to request a driver to come early. The girl on the phone for Arizona Dispatch after putting me on hold told me she had contacted my driver and he said he could not come until the regular time over an hour away. Five minutes later after I got off the phone with her I went out into the parking lot and the driver was there waiting on me. I asked the driver if dispatch had just called him and he said no.
