I was never informed the costs of a request for name change and was never informed that the former spouse was not required to restore maiden name. Mindi even at one point asked me if my wife wanted to restore her name..mind you this was requested on my ex-wife’s original first petition for dissolution of marriage!!! Incompetent! This cost me close to $500 and it was never explained to me that it did not require an actual restoration of the maiden name. Lawyer never once offered option to meet face to face.
