My Father was a patient at this facility twice August and October 2026 He was released with Discharge Paperwork or Home Health Services in which is unsafe I would not recommend Memorial City rehabilitation Center to anyone Administration Director Scott Social Worker Kyneisha does not return phone calls is Patient staff is worst Cold food being served

My brother-in-law was a resident at this facility for 29 days, and during that time his health declined significantly. When we visited him one Saturday, there was a Code Blue situation across the hall, and unfortunately that patient passed away. When it came time to discharge my brother-in-law, his condition had deteriorated to the point that we had to call an ambulance to transport him directly to the hospital. Absolutely terrible place. It’s dirty. There is water flooding the bathrooms and bloating the walls. There was mold on the walls from the water and they just simply paint over it. They lie about the care they give. If you’re not standing over them and questioning what was done they will lie about it. Have been caught flat out lying many times about several things. I was a patient here and I was treated like famiy. Every staff member was professional, polite and comforting. I want to thank the entire staff. And I apologize for the days I wasn't pleasant. I thank all of you! I highly recommend Memorial City Nursing and Rehabilitation.

I came today to pick up friend who had spine, head and neck surgery who also has issues with his hip. He was in a wheelchair taller than myself and my husband. The staff just told him yo sign discharge papers and left him all alone no one came out with him to help him get into the car. He also asked them to help him get ready an hour before we came.
