This place is no longer active.
Avoid this place. I've had countless COVID tests in the past few years and never had to pay out of pocket. But surprise!!! Almost one year later, LevelUp sends me a $117.03 bill. Looking at the statement, in addition to the COVID test they billed me for an office visit in the amount of $150. They never completed a physical assessment of any kind; only stuck a swab up my nose and sent me on my way. This place is giving insurance fraud!
Facilities are clean, but they are misleading with insurance. They let me be seen 3x last year, accepting a copay, only to hit me with a large bill this year claiming they never took my insurance.... SO why take a copay each time. Irresponsible and disengaged front desk. The first thing you should be doing is checking insurance coverage, instead of swiping on phones and gossiping. Hopefully this can resolved.
Horrible staff, extremely rude, made me wait an hour and a half to be seen when the waiting room was empty then when they finally saw me they just suggested after two minutes of talking to me that I go to the Emergency Room and then charged me for a visit when all they did was tell me I needed to go see a real doctor. I asked if they could prescribe me medication so I could go see someone outside of an emergency room since it wasn't an emergency and they said no.
