Don't bother spending your time filling out an intake and scheduling an appointment because he won't show up. Then he'll apologize and request to reschedule but don't bother with that either because he won't respond back it's just a waste of time Response: That's too bad, I wish you would have told me that when you offered to get me rescheduled as ones health always takes priority...
