I don't recommend this provider. She is unprofessional. The provider stated incorrect information in her reply below. I never refused to fill out the paperwork. I sent her a set of completed paperwork right when asked for them on 4/18 and I thought that's what was needed. I was surprised when she sent another set of paperwork on the Saturday evening before that Monday appointment. I was out of town and I could not fill them out.
