My father was a patient at this facility for many years. He passed away this month and even though I was listed as his contact, they didn't call me when he became ill at all....they contacted me almost 12 hours after his death. I have always been listed on his intake forms- and they finally found those forms- but they said " they couldn't find my contact info". They found them after we questioned them on this matter.
